Perforating:
All part, same position, use same price gudelines as used above for numbering.
All forms being booked must be perforated to tear out of books!
Books:
Up to 100 sheets per book - $0.95 each * (see perforating charges)
Padding:
Glued edge $0.50 per pad
Shrink Wrap:
$0.40 per pad
Backside Printing: All parts must print the same copy on back. Add $20. flat fee plus $15. per 1000 sheets paper (must multiply parts x #Forms)
Non-standard Color Sequence:
Minimum 10 working days and all parts must print alike. 8-1/2 x 11 and 8-1/2 x 14 add $20. per 1000 sheets (must multiply parts x #Forms)
